1. Enter Murkwater Cave After Defeating Bloody Finger Nerijus

Enter The Cave After Defeating Nerijus	Map Location Of The Cave Entrance

If you try to go to Murkwater Cave as shown in the map above, Bloody Finger Nerijus will invade. If you wait for a while during battle, Bloody Finger Hunter Yura will invade as an ally, making it easier to defeat him. Take note that you won't be able to proceed with Yura's questline if he dies here. After defeating him, use the torch as a landmark to find the entrance and enter the Murkwater Cave.

2. Reduce Patches's HP To Half During The Battle At The End Of The Cave And Have Him Surrender

※ Note that if you kill Patches here without having him surrender, his questline progression will stop. If you kill him, the quest cannot be started unless you create a New Game+

Turn Right At The Fork And Go To The Back	Prepare For A Battle Once You Open The Chest

Turn right at the fork and walk through the mist to reach the back room once you've entered Murkwater Cave. Open the treasure chest to begin the battle with Patches. He will start to surrender when his HP is reduced to half, and you will emerge victorious. Avoid killing him because you won't be able to complete his questline if you do.

Either Option Is Fine After He Surrenders

After Patches has surrendered, you will be given the dialogue option of "forgive and forget" and "Some things cannot be forgiven." There is no significant difference except for a slight change in dialogue, so either choice is fine.

Become Hostile And Obtain A Gesture When At Low Health

If you attack Patches after the battle, you can obtain the 'Extreme Repentance' gesture when you are at low health. If you get down on your knees (Grovel For Mercy gesture) towards Patches, his competitive / hostile state will be resolved.

Obtain A Gesture When You Get Caught In The Treasure Chest Transporter Trap

If you open the another treasure chest after fighting Patches, a transporter trap will be triggered and you will be sent to Mistwood on the east side of the map. Once there, you won't be able to fast travel until you rest at a nearby Site of Grace. Afterwards, return to Patches and obtain the 'Calm Down' gesture.

3. Obtain The Dectus Medallion And Head To Volcano Manor

Head to the Volcano Manor at Altus Plateau to resume the Patches questline. You must first activate the Grand Lift of Dectus to gain access to the Altus Plateau. To activate the lift, obtain the Dectus Medallion (left and right). The route to the Volcano Manor is complicated due to its steep elevation, so please refer to the video below.

If you head to the location on the map above and approach the Rainbow Stones by the cliff, a cutscene will occur where you will be pushed off the cliff by Patches. This will give you a sense of deja vu if you have played the Souls series.

4. Pick Up The Letter In The Room After Receiving The Key From The NPC

After entering Volcano Manor, speak with the NPC (Tanith) near the first Site of Grace and select 'Join The Volcano Manor' to receive the Drawing-Room Key. Afterwards, use the key to open the second door on the left at the end of the aisle and pick up the "Letter From Volcano Manor" on the table.

5. Defeat Istvan And Report Back To Tanith

After picking up the Letter from Volcano Manor, the location of the assassination target "Old Knight, Istvan" will be displayed on the map. Head to the destination while looking at map above or in the in-game map, and investigate the Red Summon Sign to invade Istvan's world. Once you have defeated him, return to the Volcano Manor and speak with Tanith several times near the Site of Grace to report your findings.

6. Pick Up The Letter In The Guest Room And Defeat Rileigh

After reporting, head to the room where you picked up the letter earlier to find another one. When you pick up the letter, the location of your next target "Rileigh the Idle" will be displayed on the map. Head to the location as you did with Istvan and report back to Tanith at the Volcano Manor once you have defeated it.

7. Obtain The Letter From Patches And Head For The Ruin-Strewn Precipice

Get A Letter From Patches	Boss Battle Along The Way

After reporting to Tanith, talk to Patches near the entrance of the Volcano Manor to receive 'Letter To Patches'. Similar to the previous targets, you will have to go to the location of the Great Horned Tragoth as shown in the map above and defeat it. Please refer to the video below for the directions since the path towards the location is complicated and you will find Magma Wyrm Makar, a boss, along the way.

8. Defeat the Great Horned Tragoth And Report To Patches

After defeating the boss Magma Wyrm Makar, examine the Red Summon Sign in the same area and defeat Great Horned Tragoth. Afterwards, return to the Volcano Manor and talk to Patches. Select "The request is done" option to report. After that, rest at a Site of Grace and speak with Patches to receive the Magma Whip Candlestick.

9. Pick Up The Red Letter From The Guest Room Table



Pick up the letter from the table in the guest room at Volcano Manor. The name of this letter is the "Red Letter."

10. Defeat Juno Hoslow In The Mountaintops Of The Giants

After picking up the Red Letter, the location of the next target "Juno Hoslow, Knight Of Blood" will be displayed on the map of the Mountaintops of the Giants. Similar to the previous targets, head to the location and return to the Volcano Manor after eliminating the target.

11. Report To Tanith And Select "See the Lord"



Once you return to the Volcano Manor, speak with Tanith to obtain the reward Taker's Cameo. If you continue with the conversation, you will be prompted with the option "See The Lord" and if you select it, you will be taken into battle with Rykard, Lord Of Blasphemy.

12. 12. Defeat Rykard, A Boss In Volcano Manor

※ If you speak to the NPCs in the Volcano Manor after defeating Rykard, they will disappear from the Volcano Manor. Avoid defeating Rykard and stop advancing Patches' questline if you want to do other NPCs' questlines.



If you select the "See The Lord" option, you will be taken to the area where Rykard is located in the Volcano Manor. This battle will have two phases in a row, along with the God-Devouring Serpent, so make sure you are well prepared before entering. Once the boss is defeated, all NPCs in Volcano Manor will disappear. so make sure to complete all NPC quests that interest you before proceeding.

13. Speak With Tanith After Defeating Rykard



After defeating Rykard, return to the Volcano Manor and talk to Patches and Tanith. Afterwards, rest at a Site of Grace and load. Both of them will be gone from the Volcano Manor. After talking to the other NPCs, rest at a Site of Grace and they will also disappear from the Volcano Manor.

14. Head To The Shaded Castle And Talk To Patches Before The Boss

Enter the Altus Plateau and descend to the bottom of the valley in front of you. Head north and you will find The Shaded Castle. Patches is sitting on the hallway in front of the boss area of the Shaded Castle. Speak with him and he will ask you to give the Dancer's Castanets to Tanith.

Be Careful Not To Attack

Pressing the attack button after the conversation may unintentionally kill him. Since the continuation of the quest is currently unknown, be careful not to attack if you want to keep Patches alive.

15. Speak With Tanith At The Location Where Rykard Used To Be



Tanith will no longer be in the Volcano Manor since she has moved to the boss area where you fought Rykard. If you talk to her and give her the Dancer's Castanets, she will give it back. Take note that you will not receive any particular reward and the Dancer's Castanets will be gone from your inventory.

16. Fight Patches in Murkwater Cave



With the latest Patch 1.04 Update, Patches now reappears again in Murkwater Cave after disappearing from The Shaded Castle. Just like the first encounter, a battle with Patches begins when the treasure chest is opened. To win the battle, reduce Patches HP less than half to force him to surrender.

Patches Questline - Where Did Patches Go?

Check The Scenic Isle In Liurnia

If Patches is not in the Murkwater Cave, it is likely that he has moved to Scenic Isle in Liurnia. You can reach Liurnia by heading northwest from Limgrave, so go check it out in person.

Check Volcano Manor In Altus Plateau

If he is not on Scenic Isle, Patches has most likely relocated to Volcano Manor in the Altus Plateau. You can get there by heading northeast of Liurnia after obtaining the Dectus Medallion (Left and Right), so go check it out in person.

Check The Shaded Castle After Defeating Rykard

All NPCs in the Volcano Manor will vanish after Rykard is defeated, and Patches will relocate to The Shaded Castle in the Altus Plateau's northern region. Enter the Altus Plateau, descend to the bottom of the valley in front of you, and head north to reach the Shaded Castle.

When the Dancer's Castanets (received from The Shaded Castle) is given to Tanith, Patches will disappear from The Shaded Castle. He used to be missing from that points but now appears to be back again in Murkwater Cave after the Patch 1.04 update.
